Youth IDOL Competition Registration

I am the parent or legal guardian of the participant named above, and I consent to the participant's entry and participation in the Youth IDOL Competition during Metro Detroit Youth Day 2026.

I understand that competing and performing may involve risks, both known and unknown. To the extent permitted by law, I waive any claim I may have against the Boys & Girls Clubs of Greater Detroit, Metro Detroit Youth Day, its sponsors, partners, staff, and volunteers for injury, loss, or damage resulting from the participant's involvement in the Youth IDOL Competition.

I authorize Metro Detroit Youth Day staff or volunteers to contact emergency services and arrange emergency medical assistance or transportation for the participant if reasonably necessary.

I certify that the information provided on this form is accurate and understand that the participant must follow all competition and event rules.
