I, the parent or legal guardian of does hereby consent to the entry of the above person in all events conducted during Metro Detroit Youth Day 2.026.

We represent that the person entered does not suffer any disability or other conditions which would cause injury or harm to the entrant, and we do hereby further waive any claim we might have against the sponsoring organizations for any injury, loss or damage to the entrant or entrant's personal property, which may result from any activity or the event.

We further agree and grant our (my) consent to the Boys & Girls Clubs of Greater Detroit and other sponsoring organizations to cause the above entrant to be admitted to an area hospital for the purpose of receiving emergency medical services if necessary.

Metro Detroit Youth Day 2.026 and sponsoring organizations and volunteers DO NOT assume responsibility for any child registered, or not registered, under the age of 5 and over the age of 17. CHILDREN UNDER AGE 5 ARE PERMITTED TO PARTICIPATE. If items are lost, visit the Lost and Found tent.

CHILDREN MUST HAVE SOME FORM OF IDENTIFICATION ON THEM IN CASE THEY ARE LOST, INJURED, OR ILL.

YOUTH WHO ARE NOT REGISTERED WILL BE PROHIBITED FROM PARTICIPATING IN CERTAIN ACTIVITIES.
